Athletic Clearance Packet
In order to be able to participate in our sports program each student must return a completed
Athletic Clearance Packet.

Your completed packet must contain:

  • A copy of your medical card (can get this at the student store)
  • 3 athletic release emergency cards (1 white, 1 blue, 1 green)
  • The Student/Parent Acknowledgement Provisions form
  • Proof of a physical (blue form)
  • Pay Transportation Fee ($20)
You can obtain an Athletic Clearance Packet at the Student Store.
Return your completed packet to the Student Store.
