Athletic Clearance Packet In order to be able to participate in our sports program each student must return a completed Athletic Clearance Packet. Your completed packet must contain: A copy of your medical card (can get this at the student store) 3 athletic release emergency cards (1 white, 1 blue, 1 green) The Student/Parent Acknowledgement Provisions form Proof of a physical (blue form) Pay Transportation Fee ($20) You can obtain an Athletic Clearance Packet at the Student Store.
